Birth Records & Other Vital Statistics

You may obtain birth or death certificates through Klamath County Public Health if the birth/death occurred in Klamath County within the last six (6) months. Otherwise, if the birth/death occurred in Oregon you will need to contact Oregon Vital Statistics in Portland at 888-896-4988. Oregon's online forms are available here.

If the birth/death occurred in another state you may use the list below to contact that respective state's vital records department.
